When you book an appointment for a close-out visit (COV) in the hospital pharmacy please take into account the following topics:

- Please only book one appointment for one protocol number. If you want to review more than one study you have to book another appointment.

- Maximum 2 persons of the Sponsor / CRO are allowed per appointment. Please make a note in the 'remark' section' if you come with 2 persons.

- The usual duration of an appointment is 2 hours. If you need more time, please indicate this in the box 'expected duration'.

   If you want to come the whole day, please book an appointment in the morning and in the afternoon.

- If you want to cancel your appointment, please inform us by email apo_studies@uzleuven.be or call +32 (0)16 34 12 45.

   A reason for cancellation is appreciated.
