When you book an appointment for an initiation visit (SIV) in the hospital pharmacy please take into account the following topics:

- By booking this appointment, you agree that the costs associated with the initiation visit and the subsequent administrative costs will be charged from the moment of SIV. If you don't agree, please cancel the SIV early.

- When you book an appointment the application form of the trial must already be signed by the hospital pharmacy. If this is not the case the appointment will be cancelled.

- Please book only one appointment for one protocol number. If you book 2 appointments on different days one appointment will be cancelled.

- An initiation visit of a transit study isn't required for the hospital pharmacy. Please don't book an appointment for this type of study.

- Preferably an initiation visit is done remotely. Exceptionally the visit can take place in the hospital pharmacy. To be discussed with the person with whom the visit was planned. If the visit takes place in the hospital pharmacy maximum 2 persons of the Sponsor / CRO are allowed.

- The usual duration of an initiation visit is 1h30. If you need more time please indicate this in the box 'expected duration'.
